Do you snore loudly (sufficiently loud to hear it even when the door is closed, or so loud that your partner elbows you to be quiet during the night)?

Do you often feel tired, fatigue or sleepy during the day (like, for example, falling asleep while driving)?

Has anyone observed that you stop breathing or choke/remain breathless while sleeping?

4.

Do you suffer or are you receiving treatment for high blood pressure?

5.

Is your body mass index greater than 35 Kg/m2?

6.

Are you over 50?

7.

What is the circumference of your neck? (Measure around the Adam's apple)
For men, is your shirt collar 43 cm or more?
For women, is your shirt collar 41 cm or more?

8.

Sex: Male?
